What is PACE?

PACE stands for Program of All-Inclusive Care for the Elderly. Instead of navigating doctors, prescriptions, transportation, therapy and home support separately, participants have one team coordinating their care.

Eligibility is based on age, location, care needs and the ability to live in the community with PACE support. Our intake team can help families understand the details.

Getting started

A thoughtful path into care.

You don't have to figure everything out before calling. The intake process gives you and your family time to learn about PACE while the team learns about you.

Start a conversation

Call, email or send a referral. The intake team begins with a few questions and explains the program.

Meet at home

An initial in-home visit helps the team understand health concerns, daily challenges and goals.

Complete assessments

Social work and home-care assessments help determine needs and program eligibility.

Visit & enroll

Tour the PACE Center, meet the care team and, if PACE is the right fit, finalize enrollment and a start date.
